Everything you want to know about Foundayo
If you’ve been searching for information about the latest GLP-1 weight loss treatments, there is an exciting new development.
Foundayo (orforglipron) has now been authorised in the UK as a once-daily oral GLP-1 medicine for weight management. The tablets are now available for order.
Until recently, the most familiar GLP-1 medicines for weight management — including Mounjaro and Wegovy — were injections.
That’s all changing. We now have oral GLP-1 options, including Wegovy tablets and Foundayo, giving patients more choice about how their weight-management treatment is delivered.
At Scout Moor Pharmacy and Clinic in Edenfield, Bury, we’re particularly interested in what this means for people looking for professional, locally based weight management in Bury, Edenfield, Ramsbottom, Rossendale and surrounding Lancashire.
But how does Foundayo compare with oral Wegovy?
And perhaps even more importantly, could Foundayo help people maintain weight loss after Mounjaro or Wegovy?
Let’s take a look.
What is Foundayo?
Foundayo contains orforglipron, a GLP-1 receptor agonist developed by Eli Lilly, the company behind Mounjaro.
GLP-1 is a naturally occurring hormone involved in appetite regulation, feelings of fullness and blood glucose control.
GLP-1 medicines can help people:
- Feel fuller for longer
- Experience less hunger
- Reduce food intake
- Manage appetite
- Lose excess weight
- Maintain weight loss as part of longer-term treatment
Foundayo is taken once a day as a tablet. One of its most interesting practical advantages is the timing of the daily dose. Foundayo can be taken once daily at any time of day, with or without food and without food or water restrictions.
That is different from oral Wegovy and could make Foundayo easier for some people to fit into their normal routine.
Foundayo vs Wegovy tablets: how do they compare?
Both Foundayo and oral Wegovy are GLP-1 receptor agonists, but they contain different active ingredients and have different administration requirements.
| Foundayo | Wegovy tablet | |
|---|---|---|
| Active ingredient | Orforglipron | Semaglutide |
| Type | GLP-1 receptor agonist | GLP-1 receptor agonist |
| How often? | Once daily | Once daily |
| Timing | Any time of day | After an overnight fast |
| With food? | Yes | No |
| Water restriction? | No | Yes – small amount of water |
| Waiting before eating? | No | At least 30 minutes |
| Weight-loss evidence | 12.4% mean weight loss at 72 weeks at the highest dose studied in ATTAIN-1 | Oral semaglutide has also demonstrated double-digit average weight loss in OASIS 4 |
| Main practical difference | Can be taken around your normal routine | Requires a specific fasting and morning administration routine |
| Prescription medicine | Yes | Yes |
Oral Wegovy must be taken after at least 8 hours of fasting, with a small amount of water, followed by a wait of at least 30 minutes before eating, drinking or taking other oral medicines. Foundayo has no equivalent fasting requirement.
What about effectiveness?
This is where it’s important not to compare the medicines too simplistically. The effectiveness figures come from different clinical trials involving different populations and treatment protocols, rather than a direct head-to-head trial. In the Phase 3 ATTAIN-1 study, the highest studied dose of orforglipron produced approximately 12.4% average weight loss after 72 weeks. Oral semaglutide has also demonstrated substantial weight loss in the OASIS programme. So while clinical trial data can help us understand the potential effectiveness of different medicines, individual results can vary considerably.
This is why we don’t believe there is one “best” weight-loss medicine for everyone.
Foundayo vs Mounjaro
It’s also important to understand that Foundayo isn’t Mounjaro in tablet form. Mounjaro contains tirzepatide, which acts on both GIP and GLP-1 receptors. Foundayo contains orforglipron, which acts on the GLP-1 receptor. Mounjaro is administered once weekly by injection. Foundayo is taken once daily as a tablet. Mounjaro remains an extremely effective weight-management treatment.
The attraction of Foundayo is different: It provides an oral alternative for people who may prefer not to have injections.
Can Foundayo help maintain weight loss after Mounjaro?
This is perhaps the most interesting new development surrounding Foundayo. For many people, losing weight is only part of the challenge. Keeping it off is the next challenge.
If an effective weight-management medicine is stopped completely, appetite can return and weight regain may occur. This means that modern weight management increasingly needs to consider long-term treatment and weight maintenance, rather than simply focusing on the initial weight loss. A Phase 3 study called ATTAIN-MAINTAIN looked at what happened when people who had already lost significant weight with injectable GLP-1-based treatments switched to oral orforglipron. Importantly, the study included people who had previously used tirzepatide — the active ingredient in Mounjaro — as well as semaglutide, the active ingredient in Wegovy.
The results were encouraging.
After switching to orforglipron:
- Previous tirzepatide users retained approximately 74.7% of their previous weight loss
- Previous semaglutide users retained approximately 79.3% of their previous weight loss
This compared with approximately 49.2% and 37.6%, respectively, among those switched to placebo.
Why is this important?
It suggests that an oral GLP-1 could have a role in maintaining weight loss after successful treatment with an injectable GLP-1 medicine.
This is particularly interesting for people who have done well on Mounjaro but would prefer to move away from injections. However, it is important not to interpret this as meaning that everyone taking Mounjaro should switch to Foundayo. The ATTAIN-MAINTAIN study was conducted in people who had already achieved substantial weight loss and reached a plateau. It provides evidence about a potential maintenance strategy — it isn’t a universal instruction to switch treatment.
What if I want to switch from Mounjaro to Foundayo?
This is likely to be a common question as Foundayo becomes available. The good news is that there is now clinical trial evidence specifically looking at this type of transition. In ATTAIN-MAINTAIN, people who had previously received injectable tirzepatide were transitioned to oral orforglipron and were able to maintain much of their previous weight reduction.
But there isn’t a simple dose conversion between Mounjaro and Foundayo.
For example, it would not be appropriate to say: “10 mg Mounjaro equals X mg Foundayo.” They are different medicines with different mechanisms and dosing schedules. The Foundayo dose needs to be determined by our pharmacists.
When might switching from Mounjaro be worth discussing?
A switch could be considered where:
- You’ve achieved a significant amount of weight loss
- Your weight has reached a stable plateau
- You’d prefer not to continue injections
- You would prefer a daily tablet
- You want to explore a longer-term maintenance strategy
- Your clinical circumstances make a change appropriate
On the other hand, if Mounjaro is working well, you’re still losing weight, you’re tolerating it and you’re happy with the injections, there may be no reason to change simply because Foundayo is now available.
What about switching from injectable Wegovy?
The same principle applies to people taking injectable Wegovy. The ATTAIN-MAINTAIN study included people who had previously taken semaglutide and then switched to orforglipron. Those participants retained approximately 79% of their previous weight reduction after a further year of orforglipron treatment. Again, this provides encouraging evidence for oral treatment as a possible maintenance strategy after injectable Wegovy. But it doesn’t mean that switching will necessarily give a better result.
If injectable Wegovy is working well and you are happy with your treatment, staying on it may be entirely appropriate.
What if I’m taking the new Wegovy tablet?
This is slightly different.
Oral Wegovy and Foundayo are both daily tablets, but they have very different administration requirements.
Oral Wegovy needs to be taken:
- After an overnight fast of at least 8 hours
- With a small amount of water
- On an empty stomach
- With at least 30 minutes before eating, drinking or taking other oral medicines
Foundayo, by comparison, can be taken:
Once daily, at any time of day, with or without food and without water restrictions.
For some people, that difference could be significant. If you’re happy with oral Wegovy, it’s working well and you can easily manage the administration routine, there may be no reason to change. But if the fasting requirement is difficult to incorporate into your morning routine, Foundayo provides another oral option to discuss with our pharmacists.
Switching isn’t about finding an equivalent dose
This is an important point if you’re researching new oral weight loss tablets such as Foundayo and Wegovy tablets. There is no simple conversion chart. These are different medicines. Our pharmacists will consider your current treatment, how you’ve responded, your weight history, your current appetite and your treatment goals before deciding how best to transition. Foundayo itself is normally introduced through a gradual dose-escalation schedule, starting at 0.8 mg once daily, with increases through the available strengths at intervals of at least one month. That is important because “switching” doesn’t mean choosing a dose yourself.
Which weight loss tablet is right for me?
There isn’t a simple answer. The decision isn’t just about which medicine produces the biggest number on a clinical trial graph. It can also depend on:
- Your medical history
- Your current BMI
- Your previous weight-loss treatment
- How much weight you’ve already lost
- Whether your weight has stabilised
- Appetite and hunger control
- Side effects
- Your preference for tablets or injections
- How easily the treatment fits into your daily routine
- Your long-term weight-maintenance goals
For some people, Mounjaro will remain the best option. For others, injectable Wegovy may be appropriate. Some may prefer oral Wegovy. And for some patients, Foundayo could offer a particularly convenient oral alternative.
